Desprescripción: El desafío de transitar de la medicina basada en guías a la medicina centrada en la persona

El ensayo analiza la desprescripción como el proceso sistemático para retirar medicamentos inapropiados, actuando como un puente esencial entre la medicina basada en guías y un modelo centrado en la persona. Aunque las guías clínicas optimizan resultados poblacionales, su aplicación rígida en adultos mayores con multimorbilidad suele derivar en polifarmacia iatrogénica. El texto identifica barreras como la inercia terapéutica y el temor del paciente, pero destaca facilitadores como la alfabetización en salud y el uso de herramientas como los criterios STOPP/START. Para una implementación exitosa, se propone un marco integrador en tres niveles: macro (políticas y métricas de calidad), meso (equipos multidisciplinarios y soporte tecnológico) y micro (toma de decisiones compartidas). En conclusión, más allá de la simple reducción de fármacos, la desprescripción estructurada se erige como una estrategia esencial que podría favorecer la autonomía y calidad de vida, honrando el principio de primum non nocere.
