Trastornos del sueño en niños con síndrome nefrótico: Estudio transversal en un hospital pediátrico venezolano

Objetivo: Determinar la prevalencia de síntomas asociados a trastornos del sueño y caracterizar los hábitos de higiene del descanso en pacientes pediátricos con síndrome nefrótico atendidos en la consulta de Nefrología Pediátrica del Hospital de Niños Dr. Jorge Lizárraga (Valencia, Venezuela). Materiales y Métodos: Estudio observacional, analítico y transversal (mayo 2024–mayo 2025) en 70 pacientes ambulatorios de 0 a 15 años. Se aplicaron los cuestionarios BISQ (< 2 años; n=6), BEARS y la Escala de Trastornos del Sueño de Bruni (≥ 2 años; n=64), junto con un cuestionario de higiene del sueño. Se calcularon frecuencias, IC 95% (método de Wilson), prueba exacta de Fisher/Chi-cuadrado y Odds Ratio (OR). Resultados: El grupo etario predominante fue el escolar (55,7%). En lactantes (< 2 años), el colecho alcanzó el 83,3% y los despertares nocturnos el 50,0%. En pacientes ≥ 2 años (n=64), las categorías con mayor cribado positivo en la escala de Bruni fueron los trastornos de inicio y mantenimiento del sueño/insomnio (28,1%; IC 95%: 17,6–40,8) y los trastornos respiratorios del sueño (17,2%; IC 95%: 8,9–28,7). La hiperhidrosis nocturna se asoció significativamente con la edad escolar (p = 0,04). El uso de pantallas antes de dormir fue el hábito inadecuado más frecuente en la cohorte (68,6%) y mostró una tendencia de asociación con insomnio en mayores de 2 años (OR = 3,41; IC 95%: 0,89–13,1; p = 0,06). Conclusiones: Los síntomas compatibles con insomnio y trastornos respiratorios del sueño representan una comorbilidad frecuente en niños con síndrome nefrótico, acompañados de hábitos de higiene del descanso inadecuados. La implementación de herramientas de cribado validadas en la práctica nefrológica pediátrica es clave para la detección temprana y el abordaje integral de estos pacientes.
